Roles & Responsibilities

• Provide compassionate direct patient care, assisting with activities of daily living
• Take and record vital signs, accurately documenting findings in the chart
• Support nursing staff with patient transport, positioning, and basic procedures
• Maintain a clean, safe environment and follow infection control practices
• Communicate promptly and clearly with nurses and interdisciplinary team members
• Observe and report changes in patient condition to licensed nursing staff
• Build rapport with patients and families, delivering respectful and dignified care

Qualifications

• Active Licensed Nursing Assistant (LNA) credential for the practicing state or ability to obtain one quickly
• Current basic health requirements such as TB screening and immunizations as required by facility
• Comfortable working in a long-term care/rehab environment and supporting varied patient needs
• Ability to pass background check and any required drug screening
• Strong communication skills and a team-oriented attitude
• Previous experience in a patient care role is preferred but new grads of LNA programs are encouraged to apply

Education

• High school diploma or equivalent
• Completion of an approved LNA training program

Knowledge & Skills

• Hands-on patient care skills: bathing, feeding, ambulation, toileting assistance
• Accurate vital signs measurement and documentation
• Basic infection control and safety procedures
• Familiarity with electronic or paper charting systems
• Good observational skills to notice changes in patient status
• Professional, compassionate bedside manner and teamwork
