Location: Uniontown, PA

Contract Type: Per-Diem

Shift Type: Per diem, 3:00 PM - 11:00 PM (Evening shift)

Hours per Week: 40

Candidate Pay Rate: $23 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to residents, focusing on dignity and comfort
Assist with activities of daily living including bathing, dressing, and mobility
Take and report vital signs and changes in patient condition promptly
Maintain accurate, timely documentation and communicate with the care team
Support safe transfers and fall prevention initiatives during your shift
Build trusting relationships with residents and families to enhance patient experience

Qualifications

Current CNA certification (required)
Active certification or ability to be certified to practice in Pennsylvania
Prior experience in long-term care or similar setting is a plus
Basic life support or CPR certification preferred
Ability to pass background check and drug screening
Strong communication skills and a team-oriented attitude

Education

High school diploma or GED preferred; CNA certification required

Knowledge & Skills

Skilled in personal care and assisting with ADLs
Comfortable taking and recording vital signs accurately
Familiarity with infection control and safety practices
Good documentation and basic charting skills
Ability to work collaboratively with nurses and interdisciplinary teams
Compassionate bedside manner and strong observational skills
