Location: Uniontown, PA

Contract Type: Per-Diem

Shift Type: Per diem (as scheduled), days vary, 3:00 PM - 11:00 PM, Evening shift

Hours per Week: 40

Candidate Pay Rate: $21 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to residents and patients
Assist with activities of daily living including bathing, dressing, and toileting
Support safe transfers and mobility, using proper body mechanics
Monitor and report changes in patient condition to the nursing team
Document care accurately and timely in patient records
Maintain a clean, safe, and respectful environment for patients
Collaborate with nurses and therapists to support individualized care plans

Qualifications

Current CNA certification in Pennsylvania (required)
High school diploma or GED preferred
Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
Comfortable assisting with ADLs and working as part of a multidisciplinary team
Strong communication skills and a compassionate bedside manner

Education

High school diploma or equivalent
Certified Nursing Assistant (CNA) certification

Knowledge & Skills

Fundamentals of patient care and infection control best practices
Basic vital signs monitoring and reporting
Ability to follow care plans and document observations accurately
Teamwork, time management, and reliable attendance
Respect for patient dignity, privacy, and safety
