Roles & Responsibilities

• Provide direct patient care with compassion and clinical accuracy
• Administer medications and treatments per facility policy and physician orders
• Monitor patient status and communicate changes clearly to the care team
• Document assessments, interventions, and patient responses accurately
• Assist with care planning and collaborate with interdisciplinary staff
• Support patient education and family communication to improve outcomes
• Maintain a safe, clean environment and follow infection control standards

Qualifications

• Active LPN license valid in New Mexico (LPN) or active RN license if applying as RN
• Current Basic Life Support (BLS/CPR) certification
• Clinical experience in a skilled nursing/rehabilitation or similar care setting preferred
• Ability to pass facility background checks and required screenings
• Strong communication skills and a patient-first attitude

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Patient assessment and vital sign monitoring
• Familiarity with documentation standards and electronic charting
• Ability to work independently and as part of a multidisciplinary team
• Good time management and prioritization during busy shifts
• Compassionate bedside manner and strong interpersonal skills
