Location: Kane, PA

Contract Type: Perm
Shift Type: Rotating nights, 6:00 PM - 6:00 AM, Two days on / Two days off / Three days on (12-hour night shifts), 84-hour biweekly pay period, includes weekends
Hours per Week: 36
Bill Rate: $26.00/hour

Roles & Responsibilities
- Supervise and direct nursing assistants to ensure residents receive high-quality care according to the plan of care
- Administer and accurately document medications, treatments, and monitoring (e.g., glucose checks) in the EHR/PCC
- Transcribe physician orders and communicate changes in resident condition to the RN Supervisor promptly
- Review and update resident care plans with clear, time-limited goals and lead team implementation
- Complete monthly summaries, incident reports, transfer forms, and other required documentation
- Assist with physician rounds, collect lab specimens, and ensure proper lab requisitions and documentation
- Count narcotics with a second nurse at shift change and maintain medication safety practices
- Participate in orientation, training, and annual evaluations for nursing assistants and provide feedback
- Maintain a clean, safe environment, submit maintenance requests, and report safety hazards
- Support residents and families through transfers, end-of-life procedures, and routine communication
Qualifications
- Current Pennsylvania LPN license or applied for PA license (temporary work permit acceptable at time of interview)
- BLS (AHA) certification required
- New grads are welcome to apply
- Nursing home/long-term care experience preferred
- COVID-19 vaccination preferred but not required
- Ability to pass employment physical and be free of communicable disease
- Strong supervisory and communication skills, with attention to infection control and documentation standards
- Ability to accept and follow written and verbal directives and to work collaboratively with interdisciplinary teams
Education
- LPN diploma or practical nursing certificate (or equivalent)
- Current Pennsylvania LPN licensure (or evidence of application for PA license at time of interview)
Knowledge & Skills
- Medication administration and monitoring
- Patient assessment and care planning
- EHR/EMR proficiency (PCC experience noted)
- Accurate documentation and incident reporting
- Infection control and medical asepsis
- Body mechanics and safe resident handling
- Quality assurance/data collection and staff performance monitoring
- Orientation and training of new staff, plus team leadership
