Roles & Responsibilities

• Conduct comprehensive clinical screening, assessment, diagnosis, and treatment planning for clients from birth through elder
• Provide brief therapy and continuing care, addressing mental health and chemical dependency needs
• Lead and facilitate discharge planning, including placements to ALF or transitional care when appropriate
• Coordinate care with hospital teams and local community partners to ensure seamless transitions and optimal outcomes
• Serve as a client advocate, assist with complex matters (for example Power of Attorney) and provide skill-building supports
• Maintain accurate, timely, and compliant documentation (screenings, treatment plans, progress notes, required reports) in alignment with state, federal, accreditation, and billing standards
• Participate in multidisciplinary team meetings, quality management trainings, and integrate best practices into service delivery
• Assist with provider enrollment, billing duties, and required reporting to support program operations

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution
• Licensed as (or in process of becoming) LCSW, LPC, or LMFT
• Valid State of Alaska driver’s license or ability to obtain within 6 months of hire
• Must be 21 years of age
• Clinical competency required within 3 months of hire and ongoing re-assessment every three years
• Experience in discharge planning, care coordination, case management, and working with multidisciplinary teams
• Cultural competency and experience or willingness to serve Tribal health populations

Education

• Master's Degree in Social Work, Psychology, Counseling or related discipline

Knowledge & Skills

• Discharge planning and transitional placement coordination
• Interviewing, crisis intervention, and substance use assessment
• Documentation practices compliant with accreditation and billing requirements
• Case management, clinical assessment, and treatment planning
• Patient advocacy and skill-building interventions
• Working effectively within multidisciplinary teams and community partnerships
• Familiarity with provider enrollment and billing processes
