Roles & Responsibilities

• Provide compassionate, patient-centered physical therapy care in home settings
• Assess functional mobility and create individualized treatment plans
• Document care thoroughly using EPIC and ensure accurate clinical records
• Collaborate with interdisciplinary teams to support patient goals
• Educate patients and caregivers on therapeutic techniques and safety
• Complete Trinity specialty competency testing and skills checklists as required

Qualifications

• BLS certification required
• Current license/certificate verification as part of credentialing
• Comfortable using EPIC for documentation and charting
• Willingness to complete Trinity skills checklist and specialty competency testing
• Meet required health screenings and immunizations listed in the checklist

Knowledge & Skills

• Strong documentation skills in EPIC
• Home health best practices and safe, effective patient handling
• Patient education and family/caregiver coaching
• Ability to perform thorough assessments and measurable treatment planning
• Familiarity with infection control, fit testing, and related health requirements
