Roles & Responsibilities

• Treat patients with movement dysfunction or pain to restore mobility and independence and reduce fall risk
• Develop and implement individualized treatment and discharge plans that improve functional outcomes
• Document evaluations, progress notes, daily notes, discharges, and phone conversations thoroughly and timely
• Coordinate with providers, insurance companies, patients, rehab staff, and supervisors to ensure seamless care and necessary authorizations
• Assist with insurance authorization/approval documentation and follow-up
• Support a safe, clean treatment environment by helping with light cleaning, laundry, and treatment room upkeep
• Perform other duties as requested to support patient care and facility operations

Qualifications

• Current or eligible Physical Therapist license for Wisconsin (state licensure required)
• Doctor of Physical Therapy (DPT) or graduation from an accredited physical therapy program preferred
• Strong documentation skills and experience with evaluations, progress notes, and discharge summaries
• Experience or interest in pain management, fall risk reduction, mobility improvement, and discharge planning
• Excellent verbal and phone communication skills for coordination with patients, providers, and payors
• Comfort assisting with light facility tasks (cleaning, laundry) as part of a team-oriented environment
• BLS/CPR certification preferred or willing to obtain

Education

• Doctor of Physical Therapy (DPT) preferred from an accredited program
• Graduation from an accredited physical therapy program required

• Patient assessment, treatment planning, and progress evaluation
• Pain management techniques and mobility-focused interventions
• Fall risk assessment and reduction strategies
• Discharge planning and safe transitions to home or next level of care
• Accurate patient documentation including evaluations, progress notes, daily notes, and discharges
• Insurance authorization processes and clear patient/provider communication
• Hands-on treatment skills to improve functional mobility and daily living activities
