Roles & Responsibilities

• Evaluate patient function and mobility in the home setting and create person-centered care plans
• Deliver skilled therapeutic interventions to improve strength, balance, mobility and safety
• Document visits and progress thoroughly using HCHB to ensure continuity of care
• Coordinate with nurses, aides and caregivers to optimize patient outcomes
• Educate patients and families on home exercise programs, fall prevention and safe function
• Manage a typical caseload of ~5-6 patients per day while prioritizing quality of care
• Travel safely between patient homes — maintain timely scheduling and communication

Qualifications

• Licensed Physical Therapist (state license required to practice in North Carolina)
• Minimum 2 years of clinical experience — home health experience strongly preferred
• Comfortable with home health documentation workflows (HCHB experience is a plus)
• Strong patient assessment, clinical reasoning and time-management skills
• Reliable vehicle and valid driver's license required for patient visits
• Good communication skills and ability to work collaboratively with interdisciplinary teams

Knowledge & Skills

• Proficient with HCHB (Homecare Homebase) documentation system
• Skilled in assessment of mobility, balance, transfers and ADL function
• Experience developing home-based therapeutic exercise programs
• Knowledge of home safety evaluation and fall prevention strategies
• Strong clinical documentation and patient education abilities
• Ability to organize a daily route and manage time effectively between visits
