Roles & Responsibilities
- Lead and coordinate the Long-Term Care QAPI program to improve resident care and safety
- Plan, organize, and direct quality assurance activities and accreditation readiness
- Conduct chart audits, data analysis, and performance reviews to identify improvement opportunities
- Facilitate quality committee meetings, maintain minutes, and drive action plans to closure
- Coordinate and support regulatory surveys and respond to survey findings
- Perform and support Root Cause Analyses (RCA) and incident investigations including FRIs and Datix events
- Develop, review, and revise policies, procedures, and facility-wide assessments
- Coach and educate staff and leaders to integrate quality and patient safety into daily operations
Qualifications
- Current full, unrestricted, active Registered Nurse (RN) license in Alaska (required)
- Certified Professional in Healthcare Quality (CPHQ), QAPI Certified Professional (QCP), or other LTC quality certification obtained within the last 3 years (required)
- Basic Life Support (BLS) - American Heart Association (required)
- Clinical Competency Assessment - SEARHC (required)
- Minimum of 2 years' experience monitoring quality and regulatory requirements, planning and coordinating process changes
- Demonstrated leadership facilitating cross-functional teams and working collaboratively
- Willingness and ability to travel a minimum of 30% as part of the role
Education
- Active Registered Nursing license in Alaska (explicit requirement)
- CPHQ or QCP (quality certification) required within 3 years
Knowledge & Skills
- Strong knowledge of QAPI, quality improvement methodologies, and CMS/regulatory compliance
- Experience with infection control practices and accreditation readiness across care settings
- Skilled in data analytics, chart audits, and presenting performance data
- Proficient with Microsoft Office (Word, Excel, PowerPoint)
- Excellent verbal and written communication and coaching skills
- Familiarity with RCA, Datix, FRIs, and risk management processes
- Ability to prioritize, problem-solve, and drive measurable improvement
