Roles & Responsibilities

• Perform comprehensive home assessments to evaluate mobility, function, and safety
• Develop and implement individualized treatment plans that help patients regain independence
• Deliver skilled physical therapy interventions in patients' homes with compassion and professionalism
• Document patient care thoroughly and accurately (Epic experience preferred)
• Educate patients and caregivers on home exercise programs, safe transfers, and fall prevention
• Coordinate care with interdisciplinary teams and community resources to optimize outcomes
• Complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist as required
• Maintain required health screenings and infection control practices to protect patients and team members

Qualifications

• Valid Physical Therapist license with ability to verify licensure/certification
• BLS certification (required)
• Epic experience preferred but not required
• Completion of Trinity competency testing and skills checklist as part of onboarding
• Current personal health requirements: immunizations and screenings per employer policy (MMR, Hep B, COVID-19, 2-step PPD or IGRA, Tdap, Varicella, etc.)

Knowledge & Skills

• Home Health care processes and standards
• Hands-on physical therapy assessment and treatment techniques
• Patient and caregiver education for safe home recovery
• Clinical documentation best practices (electronic health records — Epic preferred)
• Strong communication, time management, and care coordination skills
• Infection control, fit testing, and adherence to health screening protocols
