Roles & Responsibilities

• Conduct thorough SLP evaluations to assess patient communication and swallowing needs
• Provide daily treatments tailored to each resident's goals and functional status
• Develop and execute discharge planning to ensure safe transitions and continuity of care
• Collaborate with the therapy team and nursing staff to coordinate care and document progress
• Meet productivity expectations and follow CRG policies and procedures to support quality care

Qualifications

• 1-3 years of SLP experience as listed in the job posting
• Prior SNF experience strongly preferred (skilled nursing facility background is a plus)
• Willingness to work on-site and be present in the building to receive guaranteed hours
• Ability to follow company policies and productivity expectations (90% productivity requirement noted)
• Comfortable adhering to facility dress code (black scrubs as noted)

Knowledge & Skills

• Experience performing SLP evaluations in a skilled nursing setting
• Skilled in providing daily speech, language, cognitive and/or swallowing treatments
• Knowledge of discharge planning processes and care coordination
• Familiarity with SNF workflows and documentation practices
• Team-oriented communication and collaboration skills
