Roles & Responsibilities

• Perform thorough assessments and develop individualized treatment plans for veterans residing in the long-term care facility
• Deliver skilled therapeutic interventions—gait training, mobility programs, balance and strength restoration—to improve quality of life
• Collaborate with nurses, aides, occupational therapists, and physicians to coordinate holistic, patient-centered care
• Document progress clearly and timely in the medical record to ensure continuity and regulatory compliance
• Educate residents and families on safe mobility techniques, fall prevention, and functional independence strategies
• Supervise and mentor rehab aides and support staff to maintain high standards of care
• Participate in discharge planning and transitions to outpatient or home health services when appropriate
• Maintain infection control practices and adhere to facility policies and Veteran-focused care standards

Qualifications

• Current, unrestricted Physical Therapist license (Montana license preferred or eligibility to obtain)
• Clinical experience working with geriatric or long-term care populations preferred
• Strong assessment and documentation skills with knowledge of rehab best practices
• Excellent communication and teamwork skills for collaborating across disciplines
• Current CPR/BLS certification (or willingness to obtain upon hire)
• Professional, compassionate approach to working with veterans

Education

• Graduation from an accredited Physical Therapy program (Doctor of Physical Therapy preferred)

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, gait training, transfers and ADL-focused interventions
• Knowledge of fall prevention strategies and adaptive equipment recommendations
• Familiarity with rehab documentation standards and electronic health record systems
• Ability to prioritize caseloads and manage time efficiently in a busy long-term care setting
• Strong patient education and family communication abilities
