Roles & Responsibilities

• Provide direct patient care as directed by the Physical Therapist, delivering hands-on treatment that helps residents regain function
• Record daily treatment notes and weekly progress notes in accordance with PT Board standards to ensure accurate clinical documentation
• Assist in maintaining a safe, clean, and well-organized treatment area and equipment for optimal patient care
• Participate in Patient Care and Rehabilitation Conferences to share progress and plan coordinated care
• Follow physician and therapist treatment plans to deliver consistent, goal-driven therapy
• Train and support nursing staff and restorative aides to reinforce therapeutic programs and safe handling techniques
• Deliver caregiver and family education to support discharge planning and ongoing maintenance programs
• Communicate patient progress, concerns, and plan adjustments clearly with supervisors and interdisciplinary team members
• Report equipment issues promptly so tools are maintained in good working order
• Engage with patients, families, and colleagues professionally and compassionately to foster a positive care environment

Qualifications

• Licensed Physical Therapy Assistant in the state of California (must maintain license in good standing)
• Proficient computer skills and experience with Casamba and PointClickCare for documentation
• Strong documentation skills including treatment notes and progress notes per PT Board requirements
• Experience providing direct patient care and following therapist/physician treatment plans
• Ability to instruct and train restorative aides, caregivers, patients, and families
• Excellent communication and teamwork skills; able to collaborate with the interdisciplinary team
• Willingness to complete required continuing education to meet state practice requirements

Education

• Licensed Physical Therapy Assistant with required state board credentials and ongoing continuing education

Knowledge & Skills

• Physical therapy treatment techniques for rehabilitation and restorative care
• Patient assessment and implementation of treatment plans
• Clinical documentation standards and regulatory compliance per PT Board
• Hands-on direct patient care and safe patient handling
• Care coordination and effective clinical communication
• Familiarity with electronic health record systems such as Casamba and PointClickCare
• Equipment maintenance and basic troubleshooting to keep the department running smoothly
• Ability to lead engaging in-service trainings and caregiver education sessions
