Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to help them regain independence
• Develop and implement individualized treatment plans focused on functional goals
• Collaborate with an interdisciplinary team to coordinate seamless patient care
• Document care accurately and in a timely manner according to facility and regulatory standards
• Participate in care conferences and contribute to quality improvement initiatives
• Communicate progress and recommendations clearly with residents, families, and staff

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical skills in evaluation, treatment planning, and hands-on therapy
• Excellent communication and teamwork abilities
• Skilled nursing or post-acute care experience preferred
• Dependable, flexible, and committed to compassionate, high-quality care
