Roles & Responsibilities

• Perform thorough patient evaluations and design individualized treatment plans
• Provide hands-on physical therapy interventions to improve mobility and strength
• Educate patients and families on therapeutic exercises and safe movement techniques
• Recommend adaptive equipment and home modifications to support independence
• Collaborate with OT, nursing, and physicians to coordinate care and optimize outcomes
• Monitor progress, adjust treatment plans, and document outcomes accurately
• Ensure care plans meet regulatory standards and facility policies

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in California
• Experience in a skilled nursing facility (SNF) or similar healthcare setting preferred
• Strong clinical knowledge of therapeutic modalities and adaptive equipment
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work independently and as part of an interdisciplinary team
• Compassionate, patient-focused approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy (from an accredited program)
