Roles & Responsibilities

• Verify and enter medication orders accurately to ensure patient safety
• Check and dispense medications while following state and federal pharmacy laws
• Provide clinical patient-care services across inpatient and ambulatory settings
• Rotate into ambulatory roles such as anticoagulation clinic or oncology as assigned
• Collaborate positively with providers, nurses, pharmacy techs and other care team members
• Document interventions and communicate medication-related recommendations clearly
• Support pharmacy operations with a focus on safe, effective medication use for all ages

Qualifications

• Current Pennsylvania Pharmacist License or eligibility for reciprocity within 90 days
• Bachelor of Science in Pharmacy required (Doctor of Pharmacy preferred)
• Recent hospital pharmacy experience preferred; ambulatory rotation experience a plus
• Pharmacy residency preferred if rotating into ambulatory clinics
• American Heart Association BLS for Healthcare Provider if assigned to Anticoagulation Clinic (and keep it current)
• Strong communication skills and ability to work collaboratively across teams
• Advanced practice certifications preferred but not required

Education

• Bachelor of Science in Pharmacy (required)
• Doctor of Pharmacy (PharmD) preferred

Knowledge & Skills

• Clinical pharmacy expertise and ambulatory pharmacy experience
• Order entry/verification and medication checking/dispensing proficiency
• Working knowledge of current state and federal pharmacy laws and regulations
• Comfort caring for diverse patient populations (geriatrics to pediatrics)
• Ability to interact professionally with medical staff, vendors, and patients
• Attention to detail and commitment to medication safety
