Roles & Responsibilities
- Establish and manage collaborative practice programs with oncologists to optimize chemotherapy and supportive care for inpatient and outpatient populations
- Design and deliver staff development programs and clinical education for pharmacy, nursing, and medical teams
- Serve as the oncology drug information expert, answering clinical questions and ensuring care plans align with national guidelines
- Round with Oncology Fellows, Nurse Practitioners, and Physicians to recommend medication selections, dose adjustments, monitoring strategies, and supportive care
- Provide patient-specific medication education, using teach-back for oral oncolytics and IV chemotherapy when needed
- Review and update oncology care plans in the Electronic Health Record to maintain guideline-consistent therapy
- Identify and evaluate medication-related problems using clinical data, history, and labs, and recommend safe, effective solutions
- Precept pharmacy residents and students, supporting the next generation of oncology clinicians
- Participate actively in Oncology Service Line meetings to drive safety, quality, and cost-effective care improvements
- Promote rational, cost-conscious drug therapy and pursue opportunities for cost avoidance and savings within the Oncology Service Line
- Perform other duties as assigned that fall within the scope of the role and support outstanding cancer care
Qualifications
- Doctor of Pharmacy (PharmD) required
- Active Pharmacist license (able to practice in Delaware)
- Board Certified Oncology Pharmacist (BPS) required
- Minimum three years of hospital experience plus three years of oncology practice experience
- Completion of PGY-1 residency or equivalent clinical practice experience, or PGY-2 residency in Oncology preferred
- Strong clinical training with proven experience in chemotherapy and supportive care
- Experience precepting pharmacy residents and students is preferred
Education
- Doctorate in Pharmacy (PharmD)
- Postgraduate training: PGY-1 residency or equivalent clinical experience, or PGY-2 oncology residency (preferred)
- At least three years of clinical pharmacy practice experience; oncology experience required as noted
Knowledge & Skills
- Deep knowledge of oncology pharmacotherapy, chemotherapy administration, and oral oncolytics
- Clinical pharmacy skills including guideline-based therapy and supportive care management
- Proficiency reviewing and managing care plans within Electronic Health Records
- Strong drug information and medication education capabilities
- Ability to round with multidisciplinary teams and make evidence-based recommendations
- Experience with cost avoidance and evaluating medication-related economic impacts
- Precepting and teaching skills for residents and students
