Shift Type: Monday-Friday (5x8 Days), 7:30 AM - 4:00 PM (Day shift). Note: shift times may vary and can include 7:00 AM - 3:30 PM, 8:00 AM - 4:30 PM or 8:30 AM - 5:00 PM as scheduled. Alternating weekends required (every other weekend or 4 weekend days in a 4-week schedule); float within case management to meet facility needs.

Roles & Responsibilities

• Perform comprehensive case management and utilization review to support safe, timely discharges
• Coordinate care across disciplines — work closely with social services, PT/OT, pharmacy, and unit teams
• Develop and manage individualized plans of care and discharge plans that prioritize patient outcomes
• Perform needs assessments and coordinate DME ordering to ensure patients have what they need at home
• Use Epic (including Epic Hardstop workflows) to document care, track utilization and communicate with the care team
• Apply Milliman Guidelines and CMS/HIPAA regulations to support compliant utilization review
• Communicate with patients, families and community partners to facilitate transitions of care
• Float within case management services as needed to cover gaps and ensure continuity of care

Qualifications

• AK Registered Nurse license in hand required for Auto Offer (screen for pending license — license in hand strongly preferred)
• Minimum 1 year of nursing experience (acute care experience required)
• Epic EMR experience required (Epic Hardstop experience preferred)
• ACM/ACM-RN certification preferred
• BLS preferred
• Ability to work alternating weekends and float within case management scope
• Must have a permanent address at least 50 miles from the facility (locals not accepted for this assignment)
• Comfortable working in an acute hospital environment (Magnet, Level II Trauma)

Knowledge & Skills

• Strong knowledge of case management and utilization review processes
• Familiarity with Milliman Guidelines and CMS regulatory requirements
• Care coordination and discharge planning expertise
• DME needs assessment and ordering experience
• Proficient documentation skills in Epic (EMR) and comfort with Epic hardstops
• Experience working with diverse patient age groups (neonates through geriatrics)
• Comfortable using Microsoft Office and Microsoft Teams for communication and documentation
