Roles & Responsibilities
- Provide compassionate, hands-on nursing care to assigned patients
- Administer medications and treatments accurately and safely
- Monitor and document patient conditions, reporting changes promptly
- Assist patients with activities of daily living while encouraging independence
- Collaborate with RNs and interdisciplinary team members to support care plans
- Maintain clear, timely charting in the patient record and follow facility protocols
- Observe infection control and safety standards to protect patients and staff
- Contribute to a positive patient experience through respectful communication and advocacy
Qualifications
- Current LPN license in Pennsylvania (required)
- Graduate of an accredited Practical Nursing (LPN) program
- Current BLS/CPR certification or ability to obtain upon hire
- Ability to pass background check and drug screen as required by facility
- Strong communication, organizational, and teamwork skills
- Previous experience in long-term care or similar setting is a plus
Education
- Graduate of an accredited Practical Nursing program (LPN)
Knowledge & Skills
- Medication administration and documentation
- Vital signs assessment and basic clinical observations
- Familiarity with electronic medical records/charting
- Infection control principles and safe patient handling
- Patient education and family communication
- Time management and prioritization during busy shifts
- Ability to work collaboratively within a multidisciplinary team
