Roles & Responsibilities

• Prioritize and perform Start of Care visits to establish patient plans and ensure timely access to therapy
• Supervise Physical Therapy Assistants to maintain high-quality, safe patient care
• Report Start of Care visits the next day as required to ensure clinical oversight and continuity
• Complete clinical notes and documentation within 48 hours of each visit to maintain accurate records
• Maintain consistent, timely communication with your supervisor, intake team, and office staff
• Manage a daily caseload that averages an 8-hour working day and aligns with weekly productivity expectations
• Use Kinnser documentation system efficiently and reliably for all charting needs

Qualifications

• Active CA PT License (required)
• Minimum one year of Home Health experience (required)
• Demonstrated ability to supervise Physical Therapy Assistants
• Comfortable with mobile documentation tools (iPad) and using Kinnser
• Strong clinical documentation skills and timely reporting habits (notes within 48 hours)
• Reliable transportation and ability to travel within Solano County and expand territory as needed
• Willingness to work the assigned schedule (Saturday–Wednesday) and adapt start times to patient visits

Knowledge & Skills

• Skilled in Physical Therapy evaluation and treatment in the home health setting
• Experienced with Start of Care processes and OASIS documentation
• Proficient in clinical documentation and using electronic systems (Kinnser)
• Able to supervise and mentor Physical Therapy Assistants
• Strong communication and coordination skills with multidisciplinary teams
• Comfortable managing caseloads and meeting productivity expectations
