Roles & Responsibilities

• Provide skilled occupational therapy services to residents in a skilled nursing facility setting
• Assess patients thoroughly and develop individualized treatment plans to improve function and independence
• Document interventions and progress in line with CRG policies and facility requirements
• Collaborate closely with the interdisciplinary therapy team to deliver cohesive, person-centered care
• Meet productivity expectations (90%) while maintaining high-quality clinical documentation
• Be present in the building to receive guaranteed hours and ensure continuity of care for residents
• Communicate professionally with facility leadership, families, and the Regional Director of Therapy as needed

Qualifications

• Licensed or eligible Occupational Therapist (state license or ability to obtain licensure in NM)
• 1–3 years of clinical experience (SNF experience strongly preferred)
• Comfortable working in a skilled nursing facility environment and collaborating with a therapy team
• Willingness to follow CRG policies and productivity expectations
• Ability to be on-site as required (therapist must be present in the building to receive guaranteed hours)
• Professional appearance consistent with facility dress code (black scrubs)

Education

• Degree in Occupational Therapy (e.g., BS/Entry-level degree or Master's in Occupational Therapy) or equivalent clinical qualification

Knowledge & Skills

• Strong knowledge of SNF therapy practices, resident care, and discharge planning
• Skilled in patient assessment, ADL training, mobility and adaptive equipment recommendations
• Proficient documentation skills and familiarity with therapy productivity metrics
• Team-oriented communication and collaboration skills
• Time management and caseload prioritization in a busy skilled nursing setting
