Roles & Responsibilities

• Provide speech-language pathology evaluation and treatment to residents in a skilled nursing facility
• Develop individualized treatment plans and document progress thoroughly
• Collaborate closely with the therapy team and facility staff to support resident outcomes
• Maintain required productivity standards and follow CRG policies and procedures
• Be present onsite during scheduled hours to ensure continuity of care and guaranteed hours
• Communicate effectively with families and interdisciplinary team members to advocate for residents

Qualifications

• Certified/credentialed SLP or equivalent credentials as required by the facility
• 1-3 years of experience working in therapy, preferably with skilled nursing facility patients (explicit requirement: 1-3 years)
• Ability to follow facility policies and meet a 90% productivity requirement
• Willingness to wear black scrubs as per facility dress code and be onsite to receive guaranteed hours
• Strong documentation and interpersonal skills for resident care and team collaboration

Knowledge & Skills

• Experience with skilled nursing facility workflows and resident-centered therapy
• Assessment and treatment planning for speech, language, cognition, and swallowing disorders
• Accurate documentation and productivity tracking
• Team collaboration skills with therapists, nursing, and support staff
• Ability to adapt therapy to a SNF practice setting and follow company policies
