Roles & Responsibilities

Provide compassionate direct patient care in accordance with facility policies
Administer medications and treatments accurately and document clearly
Monitor patient status and report changes to the care team promptly
Collaborate with nurses and interdisciplinary staff to support care plans
Maintain accurate and timely documentation of patient assessments and interventions
Support a safe, clean, and therapeutic environment for residents

Qualifications

Valid LPN license (practical nurse) — active and in good standing
Current Basic Life Support (BLS) certification or willing to obtain
Comfortable working 12.5-hour shifts and delivering hands-on patient care
Strong communication skills and a team-oriented attitude
Prior experience in long-term care, skilled nursing, or similar settings preferred
Ability to pass required background checks and facility onboarding

Education

Completion of an accredited LPN/LVN program
High school diploma or GED required; additional coursework a plus

Knowledge & Skills

Clinical assessment and basic nursing interventions
Medication administration and documentation
Infection control and safety protocols
Time management and ability to prioritize patient needs
Effective communication with patients, families, and care teams
Familiarity with electronic charting or willingness to learn facility systems
