Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, patient-centered care to residents in a skilled nursing setting
Administer medications and treatments accurately and safely
Perform thorough assessments and document patient status in the medical record
Collaborate with RNs, CNAs, therapists, and physicians to coordinate care
Monitor patients for changes and escalate concerns promptly
Assist with care planning and contribute to quality improvement initiatives
Support residents with activities of daily living while promoting dignity and independence

Qualifications

Active LPN license in Pennsylvania (required)
Graduate of an accredited LPN/LVN program
Current BLS/CPR certification (or willingness to obtain)
Previous experience in long-term care or skilled nursing preferred
Strong communication skills and the ability to work well in a team
Up-to-date immunizations and ability to meet facility onboarding requirements

Education

Graduate of an accredited practical/vocational nursing program (LPN/LVN)

Knowledge & Skills

Medication administration and basic pharmacology knowledge
Clinical assessment and documentation skills
Familiarity with infection control and safety practices
Comfortable using electronic medical records and charting systems
Ability to prioritize tasks and manage time during an 8-hour shift
Compassionate bedside manner and excellent interpersonal skills
