Roles & Responsibilities

• Provide compassionate day-to-day patient care while following care plans and facility policies
• Administer medications and treatments accurately and document them in the medical record
• Monitor and record vital signs, observe changes, and report concerns to the RN or provider
• Assist with activities of daily living to promote patient comfort and dignity
• Collaborate with the care team to coordinate smooth, effective patient care transitions
• Maintain clear, timely charting and communicate effectively with patients and families

Qualifications

• Active LPN license (Pennsylvania preferred) in good standing
• Graduate of an accredited practical nursing program or equivalent training
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfortable working 8-hour shifts and flexible to facility scheduling needs
• Strong communication skills and a team-first attitude
• Previous clinical experience preferred; new grads may be considered depending on facility needs

Education

• Graduate of an accredited Practical Nursing program; LPN diploma or certificate required

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic nursing assessments and vital signs monitoring
• Familiarity with charting and electronic health record systems
• Ability to prioritize tasks in a fast-paced clinical environment
• Patient-centered care with strong interpersonal skills
• Infection control and safety protocols
