Roles & Responsibilities

• Provide compassionate direct patient care and support daily living activities
• Administer medications and treatments safely according to orders
• Monitor and document vital signs, patient condition, and changes
• Collaborate with the nursing team to create and implement care plans
• Communicate clearly with patients, families, and multidisciplinary staff
• Maintain accurate and timely clinical documentation in the charting system
• Adhere to infection control protocols and facility policies to ensure safety

Qualifications

• Current LPN license in Pennsylvania (required)
• Graduation from an accredited LPN program or vocational nursing school
• Basic Life Support (BLS/CPR) certification or willingness to obtain
• Comfortable with medication administration and basic clinical procedures
• Strong communication skills and a team-oriented mindset
• Ability to pass facility-required background checks and screenings

Education

• LPN diploma or vocational nursing certificate (required)
• Current state LPN licensure (Pennsylvania)

Knowledge & Skills

• Proven skills in patient assessment, wound care, and medication administration
• Knowledge of infection control practices and safety protocols
• Familiarity with electronic medical records and accurate documentation
• Ability to prioritize tasks and manage time effectively on an 8-hour shift
• Compassionate bedside manner and strong interpersonal skills
