Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a long-term care setting
• Administer medications and treatments safely and accurately per facility policy
• Monitor and document patient conditions, changes, and responses to interventions
• Collaborate with nurses, therapists, and care teams to develop and implement care plans
• Support residents with activities of daily living while promoting dignity and independence
• Respond to urgent situations calmly and escalate concerns to RN/physician as needed
• Participate in shift handoffs and maintain accurate electronic or paper charting

Qualifications

• Current LPN license in the state of New Hampshire (or compact/reciprocal as allowed)
• Prior experience as an LPN in long-term care, skilled nursing, or similar environment preferred
• Basic Life Support (BLS/CPR) certification
• Strong communication skills and a resident-centered approach to care
• Ability to work evenings and every-other-weekend as required by the facility

Education

• Practical Nursing diploma or equivalent and active LPN licensure

Knowledge & Skills

• Medication administration and documentation best practices
• Skilled in wound care basics, ADL assistance, and resident assessments
• Familiarity with electronic health records (EHR) and accurate charting
• Good clinical judgment, time management, and teamwork abilities
• Comfortable working in a fast-paced long-term care environment
