Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
• Administer medications and treatments safely and accurately according to plan of care
• Monitor resident condition and communicate changes promptly to the care team
• Collaborate with nurses, therapists and support staff to deliver coordinated care
• Document assessments, interventions and resident responses clearly and professionally
• Support resident dignity and independence while promoting a safe, therapeutic environment
• Participate in shift handoffs and contribute to team problem-solving to improve outcomes

Qualifications

• Current LPN (Licensed Practical Nurse) license in good standing (PA license preferred)
• Basic Life Support (BLS) / CPR certification (or willingness to obtain)
• Comfort working in a skilled nursing/long-term care environment and collaborating with interdisciplinary teams
• Up-to-date immunizations and ability to pass standard facility background checks
• Strong communication skills, reliable attendance, and a patient-centered mindset

Education

• LPN diploma or vocational nursing certificate (or equivalent) required

Knowledge & Skills

• Clinical skills in medication administration and wound care
• Familiarity with resident care planning and documentation practices
• Ability to prioritize tasks in a fast-paced care environment
• Basic infection control procedures and safety protocols
• Compassionate bedside manner and effective communication with residents and families
