Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients guided by care plans
- Administer medications and document accurately to maintain safety and compliance
- Monitor and report changes in patient condition to the care team promptly
- Assist with daily living activities while promoting dignity and independence
- Collaborate with nurses, therapists and support staff to deliver coordinated care
- Participate in care plan meetings and contribute valuable clinical observations
- Ensure accurate charting and regulatory compliance with attention to detail
Qualifications
- Current and active LPN license for Pennsylvania
- Recent clinical experience as an LPN preferred but new grads with strong skills are welcome
- Basic Life Support (BLS) / CPR certification or willingness to obtain
- Strong communication skills and a team-first attitude
- Ability to pass background checks and meet facility onboarding requirements
Education
- Graduation from an accredited practical nursing program or equivalent
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Comfortable with documentation systems and electronic charting
- Knowledge of infection control practices and resident safety principles
- Strong observational skills with the ability to escalate concerns appropriately
- Teamwork mindset and the ability to work well in a fast-paced care environment
