Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients as an LPN
• Administer medications and treatments accurately and document per facility policy
• Monitor and report changes in patient condition to the interdisciplinary team
• Support nursing assessments and follow care plans to promote positive outcomes
• Assist with activities of daily living while encouraging patient dignity and independence
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Maintain clear, timely charting and communicate effectively with families and team members

Qualifications

• Current, unencumbered LPN license for Pennsylvania
• Clinical experience in long-term care, skilled nursing, or similar setting preferred
• Current Basic Life Support (BLS) certification or willingness to obtain
• Up-to-date immunizations and ability to pass background and health screenings
• Strong communication skills and a patient-centered attitude
• Ability to pass facility orientation and required training modules

Education

• Graduate of an approved LPN/Vocational Nursing program

Knowledge & Skills

• Solid knowledge of medication administration and documentation practices
• Ability to follow established care plans and nursing protocols
• Familiarity with infection control standards and patient safety practices
• Good time management and prioritization in a fast-paced care environment
• Comfort working both independently on assigned shifts and as part of a team
