Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care and monitor patient status
Administer medications and treatments according to orders and facility policy
Document assessments, interventions, and patient responses accurately in the chart
Collaborate with nurses, providers, and the care team to ensure continuity of care
Assist with care planning and communicate changes in patient condition promptly
Promote patient safety, infection control, and a respectful environment for residents

Qualifications

Current LPN license in Pennsylvania (required)
Graduate of an accredited Practical Nursing program
Current BLS/CPR certification (or willingness to obtain upon hire)
Comfortable working in a long-term care or similar setting; experience preferred but new grads considered
Strong communication, critical thinking, and patient-focused care skills

Education

Graduate of an accredited Practical Nursing (LPN) program
Current state LPN licensure required

Knowledge & Skills

Medication administration and monitoring
Basic wound care and infection control practices
Vital signs assessment and routine clinical observations
Electronic health record documentation and charting
Time management and ability to prioritize patient needs
Team collaboration and clear communication with patients and families
