Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive initial and ongoing assessments for home health patients
Develop and implement individualized physical therapy treatment plans
Perform start-of-care visits, especially for orthopedic patients when nursing is not ordered
Manage a daily caseload of approximately 5 visits in Tehama County
Collaborate with intake RN and office manager to coordinate care
Document patient progress accurately in HCHB (HomeCare HomeBase) system

Qualifications

Active Physical Therapist license in California
Current BLS certification
Home Health experience required (willing to consider travelers with less than 2 years if they have HH experience)
Comfortable with daily driving up to 40 miles within Tehama County

Knowledge & Skills

Proficiency with HCHB (HomeCare HomeBase) EMR system
Expertise in treating orthopedic/joint replacement and geriatric patients with comorbidities like CHF and COPD
Strong clinical assessment and care planning skills
Excellent communication and patient education abilities
