LPN/LVN (License Practical Nurse)

Location: Breckenridge, MN

Start Date: 07-06-2026

End Date: 10-05-2026

Contract Type: Travel Contract

Shift Type: Rotating 8-hour shifts: 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, 10:00 PM - 6:30 AM; every other weekend and holiday required.

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide compassionate direct nursing care to residents in a long-term care setting
Administer medications and treatments as prescribed by physicians
Monitor vital signs and report changes in patient condition to RN or charge nurse
Collaborate with CNAs and interdisciplinary team to ensure quality care
Maintain accurate documentation using PointClickCare EMR system
Support activities of daily living and promote a home-like environment

Qualifications

Valid LPN license in the state of Minnesota
Current BLS certification from the American Heart Association
Minimum 1 year of LPN experience in long-term care preferred
Ability to work rotating shifts, weekends, and holidays
Strong clinical skills and a patient-centered approach

Knowledge & Skills

Proficient in PointClickCare EMR
Knowledge of common geriatric diagnoses: CHF, COPD, Alzheimer's, Parkinson's, Diabetes
Excellent communication and teamwork skills
Ability to multitask and prioritize in a fast-paced environment
