Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy to patients in their homes, helping them regain mobility and independence after surgery, falls, or illness
Develop and implement individualized treatment plans based on patient assessments and goals
Educate patients and families on home exercise programs and safety strategies
Collaborate with a multidisciplinary team to coordinate comprehensive care
Document patient progress thoroughly using HomeCare/HomeBase EMR system
Travel within a 60-mile radius to deliver care to home health and hospice patients

Qualifications

Active North Dakota Physical Therapy license required
Current BLS certification from the American Heart Association
Minimum 1 year of recent home health physical therapy experience
Valid driver's license and auto insurance
Willingness to travel within a 60-mile radius of Valley City

Knowledge & Skills

Proficiency in HomeCare/HomeBase EMR system
Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Ability to work independently without onsite supervision
Time management and organizational skills for a home health caseload
