Roles & Responsibilities

• Provide exceptional home health physical therapy services, including start of care, recertifications, and resumption of care
• Develop and implement individualized treatment plans to help patients regain independence
• Collaborate with a dedicated team of clinicians and coordinators for seamless patient care
• Complete OASIS documentation accurately and timely
• Educate patients and families on therapeutic exercises and safety strategies

Qualifications

• Active North Dakota Physical Therapy license required
• Current BLS certification from the American Heart Association
• Minimum 2 years of home health experience preferred, but new travelers welcome
• Valid driver's license and auto insurance required
• Willingness to work within a 50-mile radius and meet CommonSpirit affiliation rules

Education

• Graduate of an accredited physical therapy program (Doctorate or Master's degree in Physical Therapy)

Knowledge & Skills

• Strong home health clinical skills including OASIS training
• Excellent communication and patient education abilities
• Comfortable using electronic health records and point-of-care technology
• Self-motivated with ability to manage a daily caseload of 5-6 visits including admissions
