Roles & Responsibilities

• Provide compassionate direct patient care to residents in a skilled nursing environment
• Administer medications and treatments accurately and timely per orders
• Monitor and document vital signs, patient status, and care provided in the medical record
• Perform wound care and basic clinical procedures within scope of practice
• Collaborate with RNs, CNAs, therapists, and interdisciplinary team members to deliver coordinated care
• Assist residents with activities of daily living and support their comfort and dignity
• Follow infection control protocols and maintain a safe care environment
• Communicate clearly and empathetically with residents and their families about care needs

Qualifications

• Current LPN / Licensed Practical Nurse credential (active or eligible in Pennsylvania)
• Completion of an accredited practical nursing program or equivalent
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable working in a skilled nursing setting and collaborating with a care team
• Professional, compassionate approach and strong communication skills
• Ability to pass facility-required background and health screenings

Education

• Graduate of an accredited Practical Nursing program (Diploma/Certificate)

Knowledge & Skills

• Clinical assessment and basic nursing procedures
• Medication administration and documentation best practices
• Wound care fundamentals and infection prevention
• Electronic medical record (EMR) charting and accurate recordkeeping
• Time management and prioritization in a busy clinical setting
• Clear communication with residents, families, and interdisciplinary staff
• Teamwork and adaptability in a dynamic care environment
