Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Yolo County
Assess and evaluate each patient's therapeutic needs and create personalized care plans
Evaluate home environments to identify safety concerns and optimize rehabilitation outcomes
Guide patients and families through therapeutic exercises, gait training, and use of assistive devices
Collaborate with physicians and interdisciplinary teams to adjust treatment plans based on patient progress
Educate caregivers on therapy techniques and support ongoing rehabilitation at home
Document clinical progress and communicate changes in patient condition to the care team

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Ability to work independently while providing excellent patient-centered care

Knowledge & Skills

Physical Therapy assessment and treatment planning
Home environment evaluation and safety assessment
Therapeutic exercise and gait training techniques
Range of motion and muscle strength evaluation
Patient and family education on rehabilitation goals
Documentation and clinical reporting skills
