Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Complete approximately 5 meaningful patient visits each day
Document care using HomeCare HomeBase EMR on the provided tablet
Collaborate with the Intake RN and Manager to coordinate patient care
Support geriatric patients managing multiple conditions such as CHF and COPD
Help patients recover from joint replacements and other orthopedic procedures

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but candidates with less than 2 years will be considered if they have Home Health experience
Comfortable traveling within the service area and managing a daily mileage of approximately 40 miles

Knowledge & Skills

Physical Therapy evaluation and treatment planning
Home Health documentation and compliance
Orthopedic and geriatric patient care
Managing patients with CHF, COPD, and multiple comorbidities
Using EMR systems such as HomeCare HomeBase
