Roles & Responsibilities

• Provide compassionate direct care to residents, delivering safe and effective nursing interventions
• Administer medications and treatments accurately and on schedule
• Monitor resident conditions, report changes, and collaborate with the care team to adjust care plans
• Document assessments, interventions, and patient responses in the medical record with clarity
• Support CNAs and other staff with clinical guidance and hands-on care when needed
• Participate in interdisciplinary meetings to ensure coordinated, person-centered care

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Basic Life Support (BLS) certification (or willingness to obtain promptly)
• Prior experience in long-term care, skilled nursing, or similar setting preferred
• Strong communication and teamwork skills with a patient-centered approach
• Ability to pass required background checks and meet facility onboarding requirements

Education

• Completion of an accredited Practical Nursing (LPN) program or diploma

Knowledge & Skills

• Medication administration and documentation best practices
• Basic nursing assessments and ability to identify changes in condition
• Familiarity with electronic medical records and accurate charting
• Infection control principles and safe patient handling
• Good clinical judgment, time management, and prioritization skills
• Compassionate bedside manner and strong interpersonal skills
