Roles & Responsibilities

• Provide compassionate, competent nursing care as an LPN to residents
• Administer medications and treatments accurately and document care thoroughly
• Monitor resident condition and communicate changes to the care team promptly
• Collaborate with RNs, therapists, and support staff to deliver a coordinated care plan
• Support residents with activities of daily living while promoting independence
• Maintain accurate clinical records and follow facility policies and procedures

Qualifications

• Licensed Practical Nurse (LPN) license—eligible or active in Pennsylvania preferred
• Current BLS/CPR certification or willingness to obtain prior to start
• Comfortable and experienced with 12-hour shift assignments
• Strong communication and teamwork skills with a resident-focused attitude
• Previous long-term care or skilled nursing experience is a plus
• Ability to pass background checks and meet facility onboarding requirements

Education

• Graduate of an accredited LPN/Vocational Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Solid medication administration and documentation skills
• Knowledge of infection control and standard nursing procedures
• Ability to assess basic resident needs and escalate concerns appropriately
• Reliable time management and prioritization during busy 12-hour shifts
• Comfort using electronic health records or paper charting depending on site
