Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide personalized physical therapy services to 5-7 patients daily in their homes
Help patients recover from surgeries, falls, and other conditions with tailored treatment plans
Support both Home Health and Hospice patients on their wellness journeys
Document care accurately using the HomeCare/HomeBase EMR system
Travel within a 60-mile radius to deliver compassionate, one-on-one care

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of previous home health experience
Valid driver's license and auto insurance
Strong PT-related skills and a passion for patient-centered care

Knowledge & Skills

Home health physical therapy expertise
Comfortable working independently in patients' homes
Familiarity with HomeCare/HomeBase EMR
Excellent time management and organizational abilities
Compassionate communication with diverse patient populations
