Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover mobility and independence after joint replacements and manage chronic conditions like CHF and COPD
Complete approximately five meaningful patient visits each day using your clinical expertise
Document care accurately using the HomeCare HomeBase EMR on your provided tablet

Qualifications

Active Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than two years experience if they have Home Health background
Comfortable traveling within the service area and managing a daily mileage of around 40 miles

Knowledge & Skills

Skilled in home health physical therapy assessments and interventions
Strong communication and patient education abilities
Proficient with EMR documentation using HomeCare HomeBase
Able to work independently while collaborating with the intake team and manager
