Roles & Responsibilities

• Evaluate patients thoroughly to determine functional status, impairments, and therapy needs
• Develop individualized treatment plans and measurable goals in collaboration with physicians and the care team
• Provide skilled physical therapy interventions across inpatient, swing bed, outpatient, and skilled nursing settings
• Document assessments, plans, progress, and discharge summaries accurately and timely in the EMR
• Communicate progress and coordinate care with interdisciplinary team members and families
• Educate patients and caregivers on home programs, safety, and strategies for optimal recovery
• Apply strong clinical reasoning to manage diverse diagnoses and modify treatment safely
• Participate in quality improvement initiatives and ongoing professional development

Qualifications

• Active California Physical Therapy License in good standing
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA Basic Life Support (BLS) certification
• 1+ year of experience in acute care, swing bed/skilled nursing, or outpatient settings
• Proficiency with EMR systems and general computer skills
• Excellent clinical reasoning, documentation, communication, and problem-solving abilities
• Commitment to ongoing professional development and adherence to California and federal PT regulations

Education

• Bachelor’s, Master’s, or Doctor of Physical Therapy (DPT) from an accredited program

Knowledge & Skills

• Patient evaluation and functional assessment

• Accurate clinical documentation and EMR use
• Strong clinical reasoning to manage varied diagnoses
• BLS and safe patient handling principles
• Experience in acute care, swing bed, skilled nursing, and outpatient settings
