Roles & Responsibilities

• Provide compassionate hospice care in patients' homes, delivering comfort-focused nursing and symptom management
• Coordinate hospice case management services, ensuring timely communication among interdisciplinary team members
• Assess patient and family needs, develop individualized care plans, and update goals as conditions evolve
• Document thoroughly and accurately in the EMR (McKesson/Netsmart experience preferred)
• Educate patients and families on disease progression, symptom control, and available community resources
• Collaborate with physicians, social workers, chaplains, and volunteers to support whole-person care
• Manage travel between rural homes safely and efficiently while maintaining a high standard of care
• Advocate for patients and families to ensure dignity and quality of life during hospice care

Qualifications

• Active RN license in Vermont (VT) or eligibility to obtain one
• Experience with hospice case management is required
• Proven experience providing hospice care in home settings
• Prior rural travel experience preferred
• Experience with McKesson and/or Netsmart EMR preferred
• Reliable transportation and ability to travel to patients throughout the service area
• Willingness to commit to the contract; candidates with no RTO requests preferred

Knowledge & Skills

• Strong clinical assessment and symptom management skills specific to hospice care
• Knowledge of hospice philosophies, end-of-life care best practices, and community resources
• EMR documentation proficiency (McKesson/Netsmart preferred)
• Excellent communication skills with patients, families, and interdisciplinary teams
• Time management and route planning for rural home visits
• Ability to provide education and emotional support to caregivers and families
