Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans that promote mobility and independence
- Provide targeted therapy focused on strength, balance, gait, and functional recovery
- Monitor resident progress, adjust plans as needed, and document outcomes clearly
- Collaborate closely with physicians, nurses, and the interdisciplinary team to coordinate care
- Educate residents and caregivers on home exercise programs, safe mobility strategies, and injury prevention
- Champion resident-centered care and contribute to a positive, rehabilitative environment
Qualifications
- Active California Physical Therapist License (required)
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program (required)
- Previous SNF, rehabilitation, or geriatric therapy experience preferred
- Strong clinical assessment and patient care skills
- Excellent communication and documentation abilities
- Comfortable working as part of a multidisciplinary care team
Education
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
Knowledge & Skills
- Expertise in mobility training and rehabilitation techniques
- Proven skills in patient evaluation and individualized treatment planning
- Knowledge of strength, balance, and functional recovery approaches
- Clear, compassionate communication with residents, families, and team members
- Accurate clinical documentation and time management
- Collaborative approach to interdisciplinary care
