Roles & Responsibilities

• Complete accurate, timely OASIS documentation in accordance with agency, state, and federal rules
• Assess patients and caregivers using the nursing process to create smart, patient-centered care plans
• Develop and communicate realistic plans of care that include patients and caregivers in decision-making
• Serve as case manager for assigned patients, managing caseloads and visit timing to meet productivity standards
• Implement medical plans and nursing interventions, including high-tech skills such as IV therapy and wound VAC management
• Maintain strict infection control practices across the plan of care
• Collaborate with LPNs and supervise HCA services within scope, delegating appropriately and reviewing documentation
• Coordinate interdisciplinary care and safe discharges with physicians, therapists, social workers, and vendors
• Educate patients and caregivers about medications, treatments, discharge instructions, and reportable conditions
• Follow McKesson processes for visit locking, follow-up notes, and visit completion; complete forms like HHABN and Medicare Notice of Non-Coverage
• Participate in peer review audits and quality improvement activities to help raise the standard of care

Qualifications

• Active Registered Nurse (RN) credential (Vermont license or eligible under applicable compact rules)
• Proven experience with OASIS documentation and home health regulatory requirements
• Comfortable and competent with IV therapy, wound VAC, and complex wound care
• Familiarity with McKesson charting and visit workflows
• Experience in home health and/or hospice care, including on-call hospice coverage
• Strong case management skills and the ability to manage productivity expectations while delivering high-quality care
• Clear communication skills for interdisciplinary coordination and patient education

Education

• Graduate of an accredited nursing program (ADN/ASN/BSN) required
