Roles & Responsibilities

• Administer medications and treatments safely and according to facility policy
• Monitor patient status and report changes promptly to the care team
• Provide compassionate, patient-centered care including assistance with activities of daily living
• Accurately document assessments, interventions, and patient responses
• Collaborate with RNs, physicians, and interdisciplinary staff to execute care plans
• Maintain infection control practices and promote a safe environment
• Support patient and family education to improve outcomes and satisfaction

Qualifications

• Current LPN license (Licensed Practical Nurse) in North Carolina or compact state eligibility where applicable
• Minimum recent clinical experience in a hospital or long-term care setting preferred
• Basic Life Support (BLS) certification required; additional certifications a plus
• Strong clinical assessment, communication, and documentation skills
• Ability to work independently on per diem assignments and integrate quickly with unit teams

Education

• Graduate of an accredited practical nursing program or equivalent vocational training

Knowledge & Skills

• Medication administration and safe documentation practices
• Clinical assessment and basic wound care
• Familiarity with electronic health records and charting
• Time management and prioritization in a fast-paced care environment
• Collaborative teamwork and clear patient/family communication
