Roles & Responsibilities

• Provide compassionate, patient-centered care to residents/patients as an LPN
• Administer medications and treatments accurately and safely
• Monitor and document patient status, reporting changes promptly to the care team
• Collaborate with RNs, providers, and support staff to implement care plans
• Perform routine assessments and support activities of daily living with dignity
• Assist with admissions, transfers, and discharge processes as needed
• Maintain accurate, timely charting and adhere to facility policies and procedures

Qualifications

• Current Licensed Practical Nurse (LPN) license (required)
• Active or current authorization to practice in New Mexico preferred
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Previous LPN clinical experience preferred; excellent observational skills required
• Strong communication skills and the ability to work as part of a multidisciplinary team
• Proof of immunizations and ability to complete facility onboarding requirements

Education

• Completion of an accredited LPN/Vocational Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Clinical skills in medication administration, wound care basics, and vital signs
• Familiarity with electronic medical records and accurate documentation practices
• Ability to prioritize tasks in a fast-paced care setting while keeping patients safe
• Strong interpersonal skills for connecting with patients, families, and colleagues
• Knowledge of infection control standards and patient safety best practices
