Roles & Responsibilities

• Provide compassionate direct patient care including vital signs, basic assessments, and charting
• Administer medications and follow physician and RN care plans accurately and safely
• Perform wound care, dressing changes, and basic procedures per facility protocols
• Document patient status, interventions, and changes thoroughly in the EHR
• Communicate clearly with patients, families, and the interdisciplinary team to coordinate care
• Monitor patients for changes and escalate concerns promptly to nursing leadership
• Support a positive patient experience by delivering respectful, patient-centered care

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Mexico (required)
• Basic Life Support (BLS/CPR) certification (required or obtain upon hire)
• Prior LPN experience in a long-term care or skilled nursing setting preferred
• Strong clinical assessment, medication administration, and documentation skills
• Reliable, professional, and able to work independently in a fast-paced setting

Education

• Graduate of an accredited LPN/Vocational Nursing program
• Current New Mexico LPN licensure

Knowledge & Skills

• Solid knowledge of medication administration and infection control practices
• Comfortable with electronic health records and accurate charting
• Good clinical judgment and ability to prioritize tasks under pressure
• Strong communication and teamwork skills with patients and interdisciplinary staff
• Basic wound care, ADL assistance, and patient safety awareness
